Medication Information
If your child needs a dose of medicine during school hours, you must first submit a completed medication form THAT INCLUDES A SIGNATURE FROM HIS/HER DOCTOR OR DENTIST. The medicine must be supplied to the nurse in the prescription container with the student’s name on it. For your convenience, we are posting our medication policy, along with a medication administration form. We are also including a form to be completed for our cafeteria staff to be aware of any special needs of your child. After downloading and completing the forms, please return them to the Nurse’s Station.
